چکیده

Introduction Ankylosing spondylitis (AS), a chronic inflammatory arthritis, primarily involves the axial skeleton and results in severe thoracolumbar kyphotic deformity. Treating AS-related kyphotic deformity typically involves one or more posterior wedge osteotomies and manipulation of the position of the spine by forceful manual extension to close them. Several types of osteotomy are available, including multiple Smith–Petersen osteotomies (SPOs) and pedicle subtraction osteotomies (PSOs), which are reportedly the two major techniques for correction of thoracolumbar kyphosis resulting from AS. However, for severe AS-related kyphosis, SPOs provide very little correction; more recently, PSOs have been widely used.

A pedicle subtraction osteotomy as an adjunctive tool in the surgical treatment of a rigid thoracolumbar hyperkyphosis; a preliminary report.

BACKGROUND CONTEXT A pedicle subtraction osteotomy can be considered as part of the surgical treatment of a symptomatic sagittal imbalance. The literature on the use of this technique is limited and thus far not applied to a rigid thoracolumbar hyperkyphosis. PURPOSE To evaluate our preliminary results of a pedicle subtraction osteotomy as an adjunctive tool in the surgical treatment of thora...

Gender of patients and level of osteotomy are predictive factors for blood loss in ankylosing spondylitis patients undergoing pedicle subtraction osteotomy.

This study is to investigate the predictive factors of blood loss in ankylosing spondylitis (AS) patients. Retrospective analysis was performed in patients with thoracolumbar kyphotic deformity secondary to ankylosing spondylitis who underwent PSO from 2008 through 2013. Patient's demographics, preoperative and postsurgical global kyphosis (GK) angle, preoperative hematologic tests and other fa...

Comparison of smith-petersen osteotomy, pedicular subtraction osteotomy, and poly-segmental wedge osteotomy in treating rigid thoracolumbar kyphotic deformity in ankylosing spondylitis a systematic review and meta-analysis

BACKGROUND This study aimed to compare Smith-Petersen osteotomy (SPO), poly-segmental wedge osteotomy (PWO) and pedicular subtraction osteotomy (PSO) in patients with rigid thoracolumbar kyphosis primarily caused by ankylosing spondylitis. The efficiency, efficacy and safety of these three osteotomies have not been compared systematically, and no illness-oriented surgical type selection strateg...

The patterns of loss of correction after posterior wedge osteotomy in ankylosing spondylitis-related thoracolumbar kyphosis: a minimum of five-year follow-up

BACKGROUND Short-term studies have demonstrated good surgical outcomes after pedicle subtraction osteotomy (PSO) in ankylosing spondylitis (AS) patients, but there is a paucity of literature focused on middle-term results, especially regarding patterns of loss of correction. The objective of this study is to assess the durability of surgical outcomes and the patterns of loss of correction in th...

The clinical relevance of the presence of bridging syndesmophytes on kyphosis correction and maintenance following pedicle subtraction osteotomy for thoracolumbar kyphotic deformity in ankylosing spondylitis: a comparative cohort study

BACKGROUND The presence of bridging syndesmophytes (BS) in spinal osteotomy region serves traditionally as one critical determinant for selection of osteotomy techniques. While nowadays the proportion of kyphotic ankylosing spondylitis (AS) patients receiving pedicle subtraction osteotomy (PSO) with yet mobile neighboring disc has seen a substantial increase. Literatures investigating the clini...
